 º 3 x 5 photo (send via email or on diskette)

 º Phenom 150 Registration

 º Jr Phenom Registration

º Registration fees or deposit (balance due by Oct 1,  2009)

Fees

• $195 Camp Fee. Fees include Reversible uniforms.

• $100 Deposit secures your spot with balance due by Oct 1, 2009

• No Personal Checks or Refunds On Deposits
