 º 3 x 5 photo (send via email or on diskette)

 º Jr Phenom Registration

º Registration fees or deposit (balance due by JUNE 4, 2010)

• $195 Camp Fee. Fees include Reversible uniforms.

• $100 Deposit secures your spot with balance due by JUNE 4, 2010
